Elbow Ligament Reconstruction (UCL & LUCL)

Overview

The elbow is stabilized by strong ligaments on each side. The ulnar collateral ligament (UCL) on the inner side resists the stress of throwing. The lateral ulnar collateral ligament (LUCL) on the outer side keeps the elbow from rotating out of place and is often injured in elbow dislocations.

When these ligaments are torn and the elbow remains painful or unstable despite rehabilitation, they can be repaired or reconstructed. In a reconstruction, a tendon graft, usually taken from the forearm or around the knee, is passed through small tunnels in the bone to rebuild the ligament. UCL reconstruction is commonly known as "Tommy John" surgery. In selected cases, a ligament repair with suture augmentation allows a shorter rehabilitation.

Who is a Candidate?

Elbow Ligament Reconstruction may be appropriate for patients who meet certain criteria. Ideal candidates include:

  • Throwing athletes with UCL tears who do not improve with rehabilitation
  • Recurrent or chronic elbow instability, including posterolateral rotatory instability
  • Instability after elbow dislocation

Recovery & Rehabilitation

A splint is followed by a hinged brace and early guided motion. After UCL reconstruction, return to competitive throwing typically takes about 12 to 18 months. After lateral ligament reconstruction, most patients return to full activities in 6 to 12 months.

What is the difference between UCL repair and reconstruction?

Repair reattaches the patient’s own ligament, often reinforced with suture tape, and has a shorter rehabilitation in selected patients. Reconstruction replaces the ligament with a tendon graft and is used when the ligament is not repairable.
