Overview
The long head of the biceps tendon runs through the shoulder joint and can become painful from tendinitis, fraying, SLAP tears or instability of the tendon in its groove. It can also tear completely.
In a biceps tenodesis, the tendon is released from its attachment inside the shoulder and reattached lower on the arm bone, outside the painful area. This relieves pain while preserving the shape and function of the biceps muscle. It can be done arthroscopically or through a small incision and is often combined with other shoulder procedures, such as rotator cuff repair. A biceps tenotomy, which releases the tendon without reattaching it, is an option for some patients.

Who is a Candidate?
Biceps Tenodesis may be appropriate for patients who meet certain criteria. Ideal candidates include:
- Biceps tendinitis that has not improved with therapy and injections
- SLAP tears, particularly in patients over about 30 to 40
- Partial or complete proximal biceps tendon tears with ongoing pain or cramping
- Biceps instability in its groove
Recovery & Rehabilitation
A sling is typically worn for a few weeks. Gentle motion begins early, and biceps strengthening is delayed for several weeks to protect the repair. Most patients return to full activity in about three to four months.
Frequently Asked Questions
What is the difference between tenodesis and tenotomy?
Tenodesis reattaches the tendon to the arm bone; tenotomy simply releases it. Tenotomy has a quicker recovery but can cause a "Popeye" bulge or cramping, so tenodesis is often preferred for younger and active patients.
